ED Treatments

ED treatments are not simple; the approach depends on the type of erectile dysfunction and the causes that initiated this condition. ED treatments begin with keeping your heart and blood vessels healthy. Your doctor can identify “risk factors” that can be changed or improved.

You may be asked to change certain eating habits, quit smoking, become more active, or stop using drugs or alcohol. You may be offered an alternative to the medications you are taking. (Never stop or change prescription medications without talking to your doctor first.)

As a part of ED treatments, your doctor may also suggest treating emotional problems. These can be due to relationship conflicts, life stressors, depression, or anxiety related to past problems with erectile dysfunction (performance anxiety).

The following options are available for direct ED treatments:

ED Treatments

Non-invasive treatments are often tried first. Most of the best-known treatments for erectile dysfunction work well and are safe. However, it is helpful to ask your doctor about the side effects that could result from each option:

  • Oral medications or pills known as phosphodiesterase type 5 inhibitors are most often prescribed in the United States for erectile dysfunction (Viagra, Cialis, Levitra, Stendra)
  • Testosterone therapy (when low testosterone is detected in blood tests)
  • Penile injections for ED
  • Intraurethral drugs (IU, Alprostadil)
  • Vacuum erection devices
  • Penile implants
  • Surgical approach to sidestep penile artery damage for some younger men with a record of severe pelvic damage. Penile vascular surgical procedure is not advised for senior men with hardened blood vessels.

Psychological counseling – is not the most effective but the most popular on the list of ED treatments.

If your erectile dysfunction is caused by stress, anxiety, or depression – or the condition creates stress and relationship strain – your doctor might suggest you or your partner visit a psychologist or counselor. Don’t be surprised; this is one of the typical ED treatments.

Oral medications for ED (PDE5 inhibitors)

PDE5 inhibitors that increase penile blood flow are the only oral medications certified in the United States by the FDA for ED treatments.

  • Viagra ® (sildenafil citrate)
  • Levitra® (vardenafil hydrochloride)
  • Cialis® (tadalafil)
  • Stendra® (avanafil)

For best results, men with erectile dysfunction take these pills about an hour or two before having sex. Drugs require normal nerve function of the penis. PDE5 inhibitors improve normal erectile responses by helping the blood flow into the penis. Use these medications as directed. About 7 out of 10 men are doing well and have better erections. Response rates are lower for diabetic and cancer patients.

If you are taking nitrates for your heart, you MUST NOT take any PDE5 inhibitors. Always talk to your doctor before using a PDE5 inhibitor to learn how it might affect your health.

Most often, the side effects of PDE-5 inhibitors are mild and often short-lived. The most common side effects are:

ICI therapy often produces a reliable erection, which goes down after 20-30 minutes or with a climax. Although penile implant placement is a risky surgical procedure, it has the highest success and satisfaction rates among erectile dysfunction treatment options.

Penile implants are devices that are placed entirely inside your body. They make a stiff penis that allows you to have regular sex. It’s excellent for enhancing uninterrupted intimacy and making relationships more spontaneous.

In IE therapy, a tiny active pellet of the drug alprostadil is placed in the urethra (the tube that carries urine out of your body). Recovery time after penile implants ED treatments:

  • Most men will go through the pain and feel relief from a narcotic painkiller for a week or two. After the first week, over-the-counter pain relievers (such as acetaminophen or ibuprofen) can be substituted for narcotic pain relievers. After surgery, discomfort, bruising, and swelling will last a few weeks.
  • In the first month, men should limit their physical activity. The surgeon will explain when and how much movement is required during the healing period.
  • Men most commonly start having sex eight weeks after surgery with penile implants. If swelling or pain persists, the use of the implant may be delayed. The surgeon or healthcare professional at the surgeon’s office will discuss how the implant is inflated and deflated.

Prosthetic surgery as an ED treatment comes with risks, and patients are counseled before surgery. In the case of postoperative infection, the implant will likely be removed. The devices are reliable, but the device or part must be surgically replaced in case of a mechanical failure. If the penile prosthesis is removed, other non-surgical treatments may no longer work.

Most men with penile implants and their partners say they are happy with the results and return to more spontaneous intimacy.

Clinical Trials and the best ED treatments available.

Several restorative or regenerative treatments are being explored for future ED treatments:

  • Extracorporeal Shock Wave Therapy (ESWT) – low-intensity shock waves aimed at fixing the erectile tissue and helping to restore natural erections.
  • Intracavernosal injection of stem cells – to help regrow cavernous tissue
  • Intracavernous injection of autologous platelet-rich plasma (APRP) – to support the regrowth of cavernous tissue

What is ED

What are the different types (and causes) of ED?

Organic Erectile Dysfunction

Organic ED involves abnormalities in the penile arteries and veins and is the most common cause of ED, especially in older men. The arterial problem is usually caused by arteriosclerosis, or hardening of the arteries, although trauma to the arteries can be the cause. Controllable risk factors for atherosclerosis — being overweight, lack of exercise, high cholesterol, high blood pressure, and smoking — can cause erectile failure many times before it progresses to affect the heart.
